What Is FTM?

An FTM (Female-to-Male) procedure includes your choice of a few surgeries to change your body from female to male:

  • Top surgery (periareolar or double incision)
  • Facial masculinization
  • BioTE Hormone Replacement Therapy (BHRT)

During the FTM top surgery, we will: 

  • Remove both your breasts with a double mastectomy 
  • Eliminating any excess skin near your breasts. 
  • Reposition your nipple and areola to a more appropriate position 
  • Making other changes to create a natural, flat, and masculine-looking chest. 

Candidates for FTM 

Most patients who come to us for FTM surgery are good candidates. That said, there are some criteria we look for in ideal candidates for this transformative procedure. Some of the requirements include: 

  • A letter from your psychiatrist (or therapist) stating you are of sound mind and good judgment 
  • Understanding that this surgical procedure is irreversible 
  • You have been diagnosed with gender dysphoria 
  • Have realistic expectations 
  • In generally good health 
  • Don’t have any medical conditions that could affect the healing process 
  • Nonsmokers

Frequently asked Questions


    When considering top surgery at The Advanced Center for Plastic Surgery in Baltimore, Dr. Beverly Fischer will examine your breasts and determine if it is best to perform top surgery using a keyhole incision (periareolar incision around the nipple) or double incision (under the breasts). The incision type is usually determined by the size of your breasts.  

    On the morning of surgery, we will prep you and take your “before” photos. You will meet with the anesthesiologist. We will administer general anesthesia, and Dr. Fischer will transform you using her specialized state-of-the-art techniques and years of expertise. 

    With the periareolar incision, incisions are made around the nipple and areola, and another circular incision is made a few inches away from the nipple. The tissue is then removed through these incisions, and the chest is tightened. 

    With the double incision method, Dr. Fischer makes horizontal incisions at the top and bottom of the pectoral muscle. The tissue is removed through those incisions, and the nipple and areola are repositioned. 


    After your FTM surgery, we’ll make you comfortable with supportive bandages and a special compression garment designed to help you heal faster. Any discomfort will also be addressed with prescription pain medication. 

    The day after, you will come to the office to be checked for bleeding and to ensure that everything is healing as it should. 

    During the first week, you’ll need to rest frequently and avoid strenuous activities. You must wear your compression garment around the clock and take care of any drains. Because of the drains, you will not be able to shower during this time. You’ll also want to limit your range of motion in your arms. 

    Once your drains are removed during the second week of recovery, you can finally take a shower! You will have to follow some specific shower instructions, but at least you’ll be able to clean yourself and your incisions. You’ll still need to wear your compression garment when not showering. At this point, you can lift up to 15 pounds. 

    During week three, you can start being more active, but be careful of overdoing it. This will be the last week for your compression garment. 

    At week 4 of your recovery, you’ll finally start to feel more like yourself as your results become fully visible. You can also return to the gym. 


    You will likely see some initial results immediately after surgery, but the swelling and bruising will hide most of the results. As you recover and heal over the next few weeks and months, you’ll be able to see your true self shine through. After about a year, your scars will fade and be almost invisible.
