Insurance

At the Advanced Center for Plastic Surgery, we do not participate with or bill insurance companies directly. All procedures are paid for out-of-pocket; however, we do offer financing options to help make treatment more accessible.

If you plan to seek reimbursement through your insurance provider, we are happy to supply the necessary documentation for you to submit independently. Please note that coverage and reimbursement are not guaranteed and vary based on your individual insurance plan.

Forms available prior to surgery:

• Diagnosis codes
• CPT (procedure) codes
• Detailed cost estimates/quotes
• Facility information, including Tax ID and Physician NPI

Forms available only after surgery:

• Super bills
• Operative reports and anesthesia notes
• Clearance documentation (if applicable)

All documentation will be securely scanned and sent directly to the email address you provide. We do not send forms to insurance companies on your behalf and cannot communicate with your insurance provider.

Note: It is the patient’s responsibility to verify insurance benefits and determine if pre-authorization or prior authorization is required for potential reimbursement. Our office does not obtain or submit pre-authorizations or prior authorizations under any circumstances.

By choosing to proceed with surgery, you acknowledge that Advanced Center for Plastic Surgery cannot guarantee insurance coverage or reimbursement.
