The Advanced Center for Plastic Surgery can complete FMLA and time off paperwork provided by your employer. Please note that a completion fee is required, and payment must be made by calling our office over the phone.
To submit your paperwork, please email the forms to acps@beverlyfischer.com with the subject line FMLA [Your Full Name].

Please allow at least 3 weeks for our team to complete your paperwork. If your request is time-sensitive, we strongly recommend sending your forms as soon as possible to avoid delays.
